“Immaculate Heart of Mary, conceived without sin, please place (name of person) in the wounds of your Son, and that His most precious blood will cover them to bring forth healing of mind, body and soul, restoring (name of person) back to the way our Heavenly Father created him/her to be. This I ask in the name of your Son, Jesus. Amen.”
